HIPAA Privacy Notice

This Notice of Privacy Practices describes how The Burg Concierge may use and disclose protected health information (PHI) and how you can access that information.

Our commitment to your privacy

We are required by law to maintain the privacy and security of PHI, provide this notice, follow its current terms, and notify affected individuals when a breach may have compromised their information.

How we may use and disclose health information

Treatment

We may use and share PHI to provide, coordinate or manage care, including with other providers involved in treatment.

Payment

We may use PHI for billing, membership payment, receipts and other permitted payment activities.

Healthcare operations

We may use PHI for quality assessment, training, auditing, licensing, business planning and administrative support.

Other permitted or required disclosures

As permitted or required by law, disclosures may be made for public health and safety; reports of abuse, neglect or domestic violence; health oversight; legal proceedings; law enforcement; coroners or medical examiners; and workers' compensation.

Uses requiring written authorization

We will obtain written permission for uses such as most marketing, the sale of PHI and most sharing of psychotherapy notes. You may revoke an authorization in writing, except where action has already been taken in reliance on it.

Your rights

• Inspect or obtain an electronic or paper copy of records.
• Request correction of information you believe is incorrect or incomplete.
• Request confidential communications by a particular method or at a particular location.
• Request restrictions on certain uses or disclosures.
• Request an accounting of certain disclosures.
• Receive a paper copy of this notice.
• Choose an authorized person to act for you.

Safeguards and changes

We use administrative, physical and technical safeguards. No transmission or storage method is completely risk-free. We may update this notice; the current version will be posted here and available on request.

Complaints

You may complain to us or to the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
